Please print and complete the Release of Records- MCHWC BH, and bring it to the Clinic or you can mail it to the address below.

To expedite the process, you may email a scanned copy of the completed and signed form to [email protected] OR fax it to MCHWC Behavioral Health at 415-339-8814.

We serve patients with Medi-Cal, private insurance and self-pay/sliding fee to meet your behavioral health needs. Not insured? We can help you enroll in insurance programs.

Case Management Services

Feeling stressed or unmotivated? Looking for community resources? Catherine Ewing, our Licensed Clinical Social Worker, can work with you one-on-one to understand your unique needs.
